The Dryden Regional Health Centre is seeking to fill the following position with a highly motivated, team-orientated professional:

Permanent Full-Time Substance Use Disorders Case Manager

The Substance Use Disorders Case Manager provides assessment, brief intervention, crisis support, and coordinated care to individuals experiencing substance use concerns, working collaboratively within an interdisciplinary team.

This position works closely with the Emergency Department as a key point of entry, guiding individuals from initial presentation through discharge and into the community by coordinating care, facilitating access to services, and supporting clients in achieving their self-identified goals.

Responsibilities:

• Conduct initial and ongoing assessments and develop individualized care plans using evidence-based approaches
• Provide brief intervention, substance use education, and supportive counselling
• Deliver ongoing case management, including monitoring client progress and adjusting care plans as needed
• Coordinate referrals and facilitate access to internal and external services, including treatment programs, withdrawal management, housing, medical care, and social supports
• Provide crisis intervention and consultation as required
• Advocate for clients and support navigation of healthcare, social, and community systems
• Maintain regular contact with clients to support engagement and continuity of care
• Participate in interdisciplinary case conferences, discharge planning, and collaborative care planning
• Attend appointments in the community, including home visits, as required
• Maintain accurate, timely, and comprehensive documentation in accordance with DRHC and program standards
• Develop and maintain relationships with community partners and external agencies
• Participate in program development, quality improvement initiatives, and professional development
• Provide education and support to clients and families as appropriate
• Uphold professional standards, ethical practice, and DRHC's Code of Conduct and Core Values

Qualifications:

• Diploma in Social Work required; degree in Social Work, Psychology, or a related field preferred

• Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or equivalent regulatory body (e.g., CRPO) preferred
• Minimum 3 years' experience working with mental health and substance use concerns
• Demonstrated experience in case management, system navigation, and client advocacy
• Strong knowledge of substance use, harm reduction, relapse prevention, and concurrent disorders
• Experience providing crisis intervention and working with individuals in complex or high-risk situations
• Excellent assessment, problem-solving, and decision-making skills
• Strong communication, interpersonal, and documentation skills
• Ability to work independently while collaborating effectively within a multidisciplinary team
• Knowledge of community resources within the region is considered an asset
• Valid Ontario Driver's License
• Demonstrates behaviours consistent with Dryden Regional Health Centre's Code of Conduct and Standards of Behaviour

• Immunization record, including proof of Covid-19 vaccination status, must be provided prior to hire
