The Dryden Regional Health Centre is seeking to fill the following position with a highly motivated, team-orientated professional:
Temporary Full-Time
Personal Support Worker- Home Support
Reporting to the Manager- Rehabilitation Services, the PSW- Home Support is a valuable member of the multidisciplinary team who provides a range of patient care services to address the physical, mental and emotional well-being of patients/families in their homes. They are responsible for providing routine care, support and assistance to patients/families recently discharged from the Dryden Regional Health Centre requiring support to manage in their home setting for 1-3 months. The PSW-Home Support participates in the planning, and implementation of patient care on an ongoing basis.
