The Dryden Regional Health Centre is seeking to fill the following position with a highly motivated, team-orientated professional:
Permanent Full-Time Housing Case Manager
The Housing Case Manager provides community-based case management and support to individuals experiencing serious mental illness, addictions and housing instability. The role works with clients to assess needs, develop individualized treatment plans, access and maintain appropriate housing, and strengthen the supports needed to live as independently as possible within the community. A key focus of the position is supporting housing stability through coordinated mental health, social, financial and community-based services.
Responsibilities:
- Conduct intakes and ongoing assessments and develop individualized treatment plans based on client needs, goals and circumstances.
- Provide case management, crisis intervention and community-based support, including home visits and assistance with activities of daily living.
- Support clients in accessing and maintaining appropriate housing, including navigation of rent supplement programs, housing grants, affordable housing options and other financial supports.
- Monitor client wellness and progress and provide ongoing follow-up to support housing stability, independence and continued community living.
- Facilitate access to medical, psychiatric, mental health, addictions and other community-based services and supports.
- Advocate for clients and assist with access to financial assistance, community resources and other services required to support stability and independence.
- Participate in coordinated discharge planning, case conferences and collaborative care planning with internal and external partners.
- Maintain accurate and timely documentation, including assessments, treatment plans, progress notes, referrals, reports and program statistics.
- Provide education and support to clients and families and strengthen connections to appropriate community resources.
- Participate in program development, community education, quality improvement and professional development activities.
Qualifications:
- Diploma in Social Work, Social Service Work, Human Services or a related discipline required; undergraduate degree in Social Work, Psychology, Human Services or a related discipline preferred.
- Registration with an applicable professional college is preferred.
- Minimum two years' experience in case management supporting individuals experiencing serious mental illness and/or complex mental health needs.
- Demonstrated knowledge of serious mental illness, addictions, concurrent disorders, chronic disease management and evidence-based approaches.
- Working knowledge of rent supplement agreements, housing grants, local affordable housing options and financial support applicable to client living costs.
- Strong assessment, counselling, crisis intervention and case management skills.
- Ability to manage a caseload independently while collaborating effectively with multidisciplinary and community partners.
- Effective organizational, communication and written documentation skills.
- Valid Ontario Driver's License
- Proficient computer skills
- Able to perform the essential duties of the job

This is a Non-Union position with a comprehensive benefits package including 4 weeks' vacation and participation in the Healthcare of Ontario Pension Plan (HOOPP).
