The Dryden Regional Health Centre is seeking to fill the following position with a highly motivated, team-orientated professional:

Permanent Full-Time Opiate Case Manager

The Opiate Case Manager provides case management, therapy and outreach services to individuals experiencing opioid, methamphetamine and other substance use concerns. The role works with clients to assess needs, develop individualized treatment plans, provide therapy and crisis intervention, and connect individuals with addiction treatment, medical, psychiatric and community supports. Services are provided both within Mental Health & Addictions Services and in the community, with a focus on supporting recovery, stability and continued community living.

Responsibilities:

• Conduct intakes and ongoing assessments and develop individualized treatment plans using appropriate therapeutic approaches.
• Provide individual and group therapy, psychoeducation, supportive counselling and relapse-prevention services.
• Provide case management, crisis intervention and outreach based on individual client needs, including services within the community and client homes.
• Connect clients with appropriate addiction, withdrawal management, residential treatment, harm reduction, medical, psychiatric and community-based services.
• Monitor client progress and provide ongoing follow-up to support recovery, wellness and stability within the community.
• Advocate for clients and collaborate with primary care, psychiatric services, community agencies and other internal and external partners.
• Participate in interdisciplinary case conferences, collaborative care planning and coordinated discharge planning.
• Maintain accurate and timely clinical documentation, assessments, referrals, reports and program statistics.
• Provide support and education to families and strengthen connections to appropriate community resources.
• Participate in program development, community education, quality improvement and professional development activities.

Qualifications:

• Degree in Social Work, Psychology, Psychiatric Nursing or related human services discipline; nursing background may also be considered.
• Current registration, or eligibility for registration, with the appropriate professional college where applicable.
• Two years of experience working with individuals experiencing serious mental illness and/or substance use concerns.
• Demonstrated knowledge of addictions, substance use treatment, harm reduction and community-based supports.
• Strong counselling, assessment, crisis intervention and case management skills.
• Ability to work independently while collaborating effectively with multidisciplinary and community partners.
• Valid Ontario Driver's License
• Proficient computer skills
• Able to perform the essential duties of the job

This is a Non-Union position with a comprehensive benefits package and participation in the Healthcare of Ontario Pension Plan (HOOPP).
