The Dryden Regional Health Centre is seeking to fill the following position with a highly motivated, team-orientated professional:

Permanent Full-Time Rapid Access Mental Health Therapist

The Rapid Access Mental Health Therapist provides timely assessment and brief, goal-oriented mental health intervention to Family Health Team patients experiencing mild to moderate mental health concerns. The role focuses on early intervention, short-term treatment and helping patients access the appropriate level of care when ongoing or specialized mental health services are required.

Responsibilities:

• Provide timely mental health assessments to determine clinical needs, level of urgency and suitability for brief intervention.
• Provide focused, goal-oriented brief therapy to support early stabilization, symptom management, skill development and improved functioning.
• Develop focused treatment goals and select therapeutic interventions appropriate to individual patient needs.
• Manage an access-focused caseload through timely assessment, intervention, discharge and transition.
• Monitor patient response to intervention and determine when discharge, transition or escalation to another level of care is appropriate.
• Identify patients whose needs extend beyond the Rapid Access service and facilitate transition to ongoing, specialized, psychiatric, crisis or other appropriate mental health services.
• Provide mental health system navigation, referrals and linkages to internal, community and external services.
• Work collaboratively with Family Health Team physicians, nurse practitioners and allied health professionals to support the identification and management of mental health concerns within primary care.
• Maintain accurate and timely clinical documentation.
• Contribute to monitoring patient access, service utilization, outcomes and overall program effectiveness.
• Participate in clinical supervision, interdisciplinary collaboration, program development and quality improvement activities.

Qualifications:

• Undergraduate Degree in an applicable health or social services discipline.
• Must be a regulated health professional in good standing with the applicable regulatory college or in the process of meeting registration requirements acceptable to the organization.
• Two years of experience working in the mental health field.
• Experience providing mental health assessment and counselling.
• Experience with brief, goal-oriented therapeutic approaches and/or primary care mental health is preferred.
• Strong mental health assessment and counselling skills.
• Knowledge of brief, evidence-informed therapeutic approaches.
• Demonstrated ability to assess risk and determine appropriate service pathways.
• Knowledge of community mental health resources and referral options.
• Ability to establish and maintain focused, short-term therapeutic goals.
• Strong collaboration and communication skills.
• Ability to work independently within an interprofessional primary care environment.
• Strong organizational and time-management skills.
• Proficient computer skills.
• Able to perform the essential duties of the job

This is a Non-Union position with a comprehensive benefits package and participation in the Healthcare of Ontario Pension Plan (HOOPP).
